Guidelines for Information Disclosure
Brightpath Financial Corporation may share information with credit bureau agencies, financial institutions, insurers, private investors, creditor life companies, etc., to assess your financial situation in relation to the requested services. Information may also be shared with others working for Brightpath Financial Corporation, but only as necessary to provide these services.
Your social insurance number may be utilized to identify you with credit bureau agencies and financial institutions for credit history file matching purposes. Additionally, your information may be used to promote Brightpath Financial Corporation services to you.
Brightpath Financial Corporation pledges not to use or disclose personal information for purposes other than those for which it was collected, except with the consent of the individual(s) or as required by law. Files are maintained for a minimum of three (3) years.

Suitability
I/We acknowledge:
- The potential for an increase in the mortgage payment of a variable rate mortgage without notice, impacting the ability to make mortgage payments.
- The possibility of prepayment penalties when terminating or prepaying a mortgage before the maturity term, and the varying prepayment options of different mortgage products.
- The potential impact of changes in employment, credit, liabilities, etc., on the ability to make future mortgage payments and the potential disqualification for the mortgage due to changes prior to closing.
- The monthly mortgage payment of PI&T amount will be discussed with my agent based on my/our current financial obligations (including applicable condo fees).
- The benefits of creditor mortgage protection.
- The inherent risks associated with mortgages. The agent has assessed my/our needs to assist in determining the most suitable mortgage, including offering support in preparing a budget.
Credit Bureau Consent
I/We confirm that the information provided is accurate and complete. I/We understand that it is used to evaluate my/our credit responsibility and determine my/our eligibility for credit. I/We authorize Brightpath Financial Corporation or their designate to obtain a credit report, recognizing that the completion of a credit application may involve additional credit reports.
I/We consent to additional credit reports for up to six (6) months from the date signed below.
I/We also authorize Brightpath Financial Corporation to share this credit information for the purpose of securing credit with potential mortgage lenders, mortgage insurers, or other service providers. Brightpath Financial Corporation retains the application and credit information.
